Continued Care with Texas Care

At Texas Care, licensed professionals may work with clients, their clinical team and support system to coordinate care in the community. With case management, clients get the help and guidance they need to connect with resources and services in the community.

What is Continued care?

Mental health continued care planning  often involves coordinating community services and connecting clients with the resources they need. In addition, a case manager may advocate for clients and monitor their community services. Family members, friends and caregivers also may receive support from a mental health case manager.

At Texas Care, we partner with clients and their support system to evaluate, monitor and coordinate community services in the areas of mental and behavioral health. Case managers may provide the following services:

What is Continued Care?

At Texas care, Continued Care is an individualized plan that’s implemented once primary treatment is complete. Your continued care plan is designed based on your specific needs and issues to help you navigate early recovery. The Substance Abuse and Mental Health Services Administration stresses that a successful aftercare plan supports ongoing recovery through evidence-based practices and a range of services that help you maintain motivation and momentum as you move forward.

Components of a Typical Continued Care Plan

A typical aftercare plan will include ongoing online therapy and participation in a community-based peer support groups. Other components are added to the plan based on your needs and may include:

    • Mental Health Rehab to help improve your chances of successful employment
    • Legal assistance
    • Educational assistance
    • Help finding safe housing
    • Ongoing monitoring of any mental or physical health problems
    • Relapse prevention programming
    • Activities that promote developing healthy relationships and engaging in enjoyable pursuits

Case Management

Recovery is hard work. You'll be assigned a Continued Care Services Coordinator that will help you step by step on your road to better living! It requires daily mindfulness and a high level of self-care. Your Coordinator helps you stay on track with your recovery plan and your short- and long-term goals.
